About Our Office & Practice
New Patients
New Patients need to arrive 15 minutes prior to your scheduled appointment time. You must have your Driver's License and all current insurance cards when you come in for your appointment. Medicare ONLY patients must pay their 20% and deductible (if applicable) at the time of the visit. Patients that are Self Pay have two options. You can pay in full with a 25% discount or $100 is due at the time of the visit with payment arrangements required at the time of the visit.
